ACKNOWLEDGEMENT

1. I acknowledge that I have reviewed and understand the Salary Advancement Credit (SAC) earning guidelines and requirements for Mentoring.

2. I acknowledge that documentation of mentoring completion requires the submission of a time log documenting each mentoring session, including the session dates, hours completed, and the total number of mentoring hours completed during the semester. Time logs must be submitted to the IPD within ten days of your last session and no later than September 1. I understand that the IPD will report earned IPD units of credits to District Human Resources by September 15 for consideration of horizontal salary advancement. Failure to submit the required documentation by the September 1 deadline may preclude horizontal movement until the following academic year, in accordance with the CFE/AFT CBA.

Awarding of final units of credit is contingent upon the IPD Chair's review and approval of documentation verifying the actual number of hours completed for the approved SAC program.

3. I acknowledge that if I have not already submitted a completed Mentor-Mentee Agreement Form, I am responsible for submitting a fully executed form, including the mentee's signature, to the IPD Office at GWCIPD@gwc.cccd.edu.
